OffMaps uses OpenStreetMaps (the Wikipedia of maps). The coverage may be different from region to region. You can have a look at http://maps.cloudmade.com to see the quality of the maps used.

If you have an iPhone: yes. If you have an iPod Touch you need to have WiFi enabled and have known networks within reach. The accuracy is best on the iPhone 3G and 3GS with GPS.
If the iPhone has had a GPS location fix not too long ago and not too far away, an unassisted fix happens not much slower than an assisted one. But, if you haven't used any location services for a few days or if you've just disembarked from an airplane, locking on to the GPS satellites takes a lot longer. Depending on how much sky is visible, this can take two to more than five minutes.
Often you will need to switch on data roaming for the first fix and can then disable it again (alternatively you can use a WiFi connection for the first localization). Please understand that this is a limitation on the iPhone and not the application.
